Eligibility & Forms

Before any student-athlete is allowed to try out or practice, there are several items that must be completed by requirement of the Florida High School Athletic Association (FHSAA).

In addition to the forms above, the FHSAA requires all student-athletes to complete the NFHS Concussions Course, Heat Illness Course, and Sudden Cardiac Arrest Course prior to participating in tryouts or practice.  Click here for instructions.
